Monday Friday, 8:00 AM 5:00 PM (40 hours/week)

Position Overview
Seeking a Full-Time NP for a unique blend of home health and telehealth care, focusing on a geriatric patient population. This role includes conducting in-home visits and virtual appointments, providing primary and chronic care management for homebound patients. Ideal for a provider experienced in comprehensive geriatric care, chronic disease management, and patient education.

Key Responsibilities
6 home visits per day (appointments approx. 45 minutes)
Up to 8 telehealth patients/day (equipment provided)
Conduct physical exams, diagnose/manage chronic illnesses, perform advanced care planning
Coordinate care with other healthcare providers
Travel across a defined regional area within 5 counties (scheduling team sets daily route)
Mix of new and established patients

Requirements
Board Certified :
Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP)
NC License and DEA :
Required
BLS :
Must be AHA or Red Cross certified
Experience :
2 3 years in acute care, long-term care, outpatient, or home health required
Palliative care experience preferred
EMR :
Athena experience preferred (will need to complete training modules if unfamiliar)
Travel :
Must have valid driver's license and personal transportation
Automobile liability insurance required
