Job Summary and Qualifications
* Demonstrates advanced clinical skills and ability to assess patients to determine appropriate level of care.
* Identifies cultural considerations, gender identity and preferred language for patients and family
* Demonstrates advance practice in assessments, and all other documentation timely and in compliance with requirements (as appropriate to scope)
* Assesses environment and patient's safety status and reports/responds to actual and potential safety issues or changes in the patients condition
* Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)
* Recognizes and acts upon unexpected assessment findings
* Leads in ensuring a safe structured milieu with evidence based treatment modalities/resources
* Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers
* Provides education and guidance regarding state commitment processes, abuse reporting requirements, duty to warn and guardianship processes.
* Assesses, collaborates, coordinates and provides guidance on complex discharge planning needs and readiness for discharge with the multidisciplinary team.
* Develops and maintains a community resource guide including access to public assistance/alternatives.
* Facilitates and participates in Community Education, Activities and Support Groups (as appropriate to scope)
* Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.
