RN, Registered Nurse – NHC Home Care Columbia

• Unencumbered current Registered Nurse license in the state where the Home Care agency is located or in a compact state, if applicable.
• Minimum one (1) year experience as a RN / professional nurse.
• Experience in home care is desirable.

• Utilize the nursing process to identify and achieve patient goals: assessment, planning, implementation, and evaluation.
• Assess the physical, psychosocial, and environmental factors that affect a patient’s health to develop a comprehensive nursing care plan that attains the patient’s desired health outcomes in a culturally appropriate manner.
• Collaborate with the interdisciplinary team to ensure personal care, medical care, and rehabilitation provide optimal patient health and well‑being.
• Teach patients and caregivers various aspects of health care and disease management appropriate to their needs, level of education, and understanding, and document continuing needs and levels of understanding.
