About the Role:
As a Night Shift LPN at Magnolia Rehab and Longterm Care, you will play a vital role in providing exceptional care to our residents, ensuring their physical, emotional, and social needs are met. You will work closely with other healthcare professionals to develop and implement individualized care plans, monitor and maintain accurate records of patient care and treatment, and maintain a clean and safe environment.

Responsibilities:
Provide direct patient care to a diverse population, including assisting with activities of daily living (ADLs), administering medications, and performing vital signs
Collaborate with other healthcare professionals to develop and implement individualized care plans
Monitor and maintain accurate records of patient care and treatment
Maintain a clean and safe environment, adhering to infection control and safety protocols
Communicate effectively with families and healthcare providers to ensure seamless care transitions
Participate in ongoing education and training to stay current with industry developments and best practices
