NHC Maury Regional Transitional Care Center is seeking a professional, organized, and compassionate Admissions Nurse to join our team.

The Admissions Nurse plays an important role in helping ensure a smooth transition for patients entering the center. This position works closely with referral sources, patients, families, nursing leadership, physicians, case managers, social workers, and other members of the interdisciplinary team to review clinical information, coordinate admissions, and help ensure incoming patients can be safely and appropriately served.

Position Responsibilities

Review clinical information and referral documentation for prospective admissions.

Help determine whether the center can appropriately meet the clinical needs of referred patients.

Communicate with hospitals, physicians, case managers, social workers, referral sources, patients, and families regarding potential admissions.

Collaborate with nursing leadership and other departments to coordinate safe and timely admissions.

Review diagnoses, medications, treatments, physician orders, and other relevant clinical information prior to admission.

Assist with ensuring required admission documentation is complete and accurate.

Coordinate important clinical information with the receiving nursing team before patient arrival.

Follow up on missing or incomplete clinical documentation as needed.

Help identify special equipment, medication, treatment, or care needs prior to admission.

Maintain accurate documentation regarding referrals, pending admissions, and admission decisions.

Provide professional and compassionate communication to patients and families throughout the admission process.

Maintain confidentiality and follow applicable NHC policies, procedures, and regulatory requirements.

Support a positive admission experience and smooth transition into the center.

Perform additional nursing and admissions-related duties as assigned.

Qualifications

Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Tennessee, or a current multistate/compact license that permits practice in Tennessee.

Graduate of an approved nursing program.

Current CPR certification as required.

Previous long-term care, skilled nursing, rehabilitation, hospital, admissions, or case-management experience preferred.

Strong clinical assessment and critical-thinking skills.

Excellent communication and interpersonal skills.

Strong organization and attention to detail.

Ability to review clinical documentation and manage multiple referrals and priorities.

Ability to work collaboratively with patients, families, referral sources, and interdisciplinary healthcare teams.
