Overview

Responsibilities
  • Direct and coordinate all nursing care based on established clinical nursing practice standards.
  • Collaborate with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes.
  • Practice nursing as described: diagnosis and treatment of human responses to health alterations. Utilize the nursing process, including administering treatments and medications, interpreting diagnostic tests, formulating a plan of care, collaborating with other providers, and educating patients and families. Pass required competencies. Exercise discretion and independent action within prescribed limits.
  • Collect patient health data and determine data collection priorities based on the patient’s condition; use appropriate assessment techniques; involve patient, family, and providers as appropriate; ensure data collection is systematic and ongoing; document data in the medical record; analyze data to identify diagnoses/problems.
  • Derive diagnoses/problems from assessment data and validate with patient and care team; document diagnoses to support outcomes and plans; identify individualized expected outcomes.
  • Derive measurable outcomes and document them; formulate outcomes with patient and providers; ensure outcomes are realistic and attainable given resources; set time frames; use outcomes to guide continuity of care and anticipate potential problems.
  • Develop an individualized plan of care with patient and care team; ensure the plan reflects current nursing practice; document and provide continuity of care; implement planned interventions.
  • Establish, perform, document, and evaluate interventions; ensure safe and appropriate delivery of care; evaluate patient progress toward outcomes and revise as needed.
  • Ensure ongoing, systematic evaluation and document patient responses; evaluate intervention effectiveness and revise diagnoses, outcomes, and care plans as needed; involve patient and care providers in evaluation; participate in quality-of-care activities as appropriate (including data collection and analysis to monitor care quality and effectiveness). Other duties as assigned.
Qualifications
  • Education:

    Bachelor’s Degree preferred; graduate of an accredited nursing program.
  • Licenses/Certifications:

    Licensure as a Registered Professional Nurse (RN).
  • Experience:

    Minimum of one year of hospital clinical experience preferred.
