Job Responsibilities
  • Identify high risk populations and tailor interventions based on the needs of the population and the resources available.
  • Provide treatments, administer medications/vaccines and provide preventive and maintenance care to patients/families consistent with nurse protocols.
  • Screen and refer clients to other community service providers.
  • Participate in the planning, organizing and evaluation of program areas to ensure the goals and objectives of the program are met.
  • Provide clinical services to target population.
  • Apply principles of population health to practice.
  • Conduct comprehensive assessments of clients’ health care needs.
  • Develop comprehensive plans of care for individuals, families and populations.
  • Modify client treatment plans as indicated by client’s responses and conditions.
  • Provide health education instruction and/or classes to individuals, families, for the promotion and maintenance of health.
  • Develop and sustain community partnerships.
  • Participate in drills and exercises on pandemic influenza, mass sheltering, and mass dispensing.
  • Perform job responsibilities with minimal supervision.
  • May manage specific program activities and/or specific program evaluations.
  • Adhere to all DCBOH policies and procedures.
Minimum Qualifications
  • Bachelor’s degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification.
  • OR
  • Associate’s degree in nursing from an accredited college or university AND one year of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR Certification.
Preferred Qualifications
  • Prior experience in a medical office setting with excellent customer service skills.
  • Public Health experience is a plus.
  • Knowledge and usage of computers and Windows Software related programs.
  • The ability to translate English to Spanish and Spanish to English orally and in writing is preferred but not required.
