Registered Nurse - Home Health

Join our dedicated home health team as a Registered Nurse, providing comprehensive, patient-centered care to adult individuals within their homes in Columbus, Georgia, and the surrounding region. This role involves managing a diverse caseload, delivering skilled nursing services, and ensuring continuity of care in a community-based setting.

• Provide, coordinate, and direct skilled nursing care to home health patients in accordance with physician orders and agency policies.
• Competently apply the nursing process for patient assessment, care planning, intervention, and evaluation.
• Manage a daily caseload of approximately 5–8 patients, equivalent to a weekly productivity target of 30 points.
• Perform a variety of nursing services including start-of-care assessments, wound care, medication management, and follow-up visits.
• Supervise and evaluate the performance of Home Health Aides and Licensed Practical/Nursing staff.
• Ensure all patient documentation is completed accurately and submitted within 24 hours of the patient visit.
• Travel daily to patient homes and periodically to the assigned care center for meetings.
• Accept all assigned patients on the schedule, regardless of travel distance between appointments.

• Minimum of one (1) full year of recent adult Home Health or Hospice experience.
• Active Registered Nurse (RN) license in Georgia (pending licenses not accepted).
• Current Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certification.
• Proficiency with Home Care Home Base (HCHB) Electronic Medical Record (EMR) system, with documented use within the last six (6) months.
• Proficiency with OASIS documentation, with documented use within the last six (6) months.
• Reliable transportation, current driver’s license, and agency-required liability insurance.
• Willingness to travel up to 75 miles from the assigned care center.

• Contract Length: 13 weeks
• Assignment Dates: June 29, 2026 – September 28, 2026
• Shift Type: Day Shift (8:00 AM – 5:00 PM)
• Days Per Week: 5 (Monday-Friday)
• On-Call Requirements: Required for one entire week per month, covering 5:00 PM – 8:00 AM on weekdays and all day Saturday and Sunday.
• Weekend Rotations: Included as part of the monthly on-call week.
