Position: LICENSED PRACTICAL NURSE

Ohio Health is seeking a dedicated LPN to join our Healthcare & Medical Services team. In this role, you will provide compassionate, patient-centered care including medication administration, basic treatments, patient education, and coordination with RNs and providers. You will document accurately in the EMR, support care plans, and contribute to a safe, inclusive environment for all patients and families. Responsibilities

  • Provide direct patient care within LPN scope under supervision of an RN and/or provider
  • Obtain and document vital signs, patient histories, and ongoing assessments
  • Administer medications and treatments safely following organizational policies and standards
  • Assist with diagnostic tests, procedures, and specimen collection
  • Monitor patients for changes in condition and promptly report concerns to the RN or provider
  • Document all care accurately and timely in the electronic health record (EHR)
  • Reinforce patient and family education on diagnoses, treatments, and discharge instructions
  • Support infection prevention and patient safety protocols at all times
  • Collaborate with the interdisciplinary team to support individualized care plans
  • Maintain a clean, organized, and stocked patient care environment
  • Participate in quality improvement, safety, and competency-based training activities
  • Demonstrate culturally sensitive, respectful communication with patients, families, and colleagues

Required Skills

  • Medication administration and reconciliation

  • Electronic Health Record (EHR) documentation (e.g., Epic or similar)
  • Wound care and dressing changes
  • Phlebotomy and specimen collection
  • IV therapy and infusion management (per scope and certification)

  • Patient and family education techniques
  • Time management and prioritization in a fast-paced clinical setting
  • Interdisciplinary team communication and collaboration
  • Understanding of HIPAA and patient privacy regulations
