Ohio Health is seeking a dedicated LPN to join our Healthcare & Medical Services team. In this role, you will provide compassionate, patient-centered care including medication administration, basic treatments, patient education, and coordination with RNs and providers. You will document accurately in the EMR, support care plans, and contribute to a safe, inclusive environment for all patients and families. Responsibilities
- Provide direct patient care within LPN scope under supervision of an RN and/or provider
- Obtain and document vital signs, patient histories, and ongoing assessments
- Administer medications and treatments safely following organizational policies and standards
- Assist with diagnostic tests, procedures, and specimen collection
- Monitor patients for changes in condition and promptly report concerns to the RN or provider
- Document all care accurately and timely in the electronic health record (EHR)
- Reinforce patient and family education on diagnoses, treatments, and discharge instructions
- Support infection prevention and patient safety protocols at all times
- Collaborate with the interdisciplinary team to support individualized care plans
- Maintain a clean, organized, and stocked patient care environment
- Participate in quality improvement, safety, and competency-based training activities
- Demonstrate culturally sensitive, respectful communication with patients, families, and colleagues
Required Skills
- Medication administration and reconciliation
- Vital signs measurement and basic patient assessment
- Electronic Health Record (EHR) documentation (e.g., Epic or similar)
- Wound care and dressing changes
- Phlebotomy and specimen collection
- IV therapy and infusion management (per scope and certification)
- Basic life support and emergency response techniques
- Infection prevention and control practices
- Patient and family education techniques
- Time management and prioritization in a fast-paced clinical setting
- Interdisciplinary team communication and collaboration
- Understanding of HIPAA and patient privacy regulations
