The ADRC Counselor is responsible for the professional assessment and determination of a level of care and appropriateness for community-based services. The clients screened may or may not be Medicaid eligible or they may or may not be eligible for services provided to the community through Legacy Link, Inc. This staff member also serves as a resource to community members in need of information and referral of information.

DUTIES

AND RESPONSIBILITIES ESSENTIAL FUNCTIONS
  • Receives and screens emergency and non- emergency referrals over the phone
  • Documents all potential referrals on screening/assessment instrument in the state data base
  • Reviews initial financial, medical, and social information of potential client as presented by referral source or the applicant.
  • Verifies Medicaid eligibility and/or screens for MAO/PMAO eligibility by using standardized guidelines of finances.
  • Explains thoroughly the scope and purpose of CCSP and/or HCBS
  • Identifies client’s needs and desired services as stated by referral source or the applicant.
  • Screens clients for HCBS, CCSP and other respite programs
  • Determines that client is ineligible or inappropriate for a level of care for CCSP placement and refers client to other appropriate resources.
  • Determines that client is eligible and appropriate for level of care for CCSP placement and refers client for full assessment for CCSP. If CCSP is not admitting new referrals, places client on waiting list.
  • Determines eligibility of client for HCBS programs, places on the wait list or refers to other appropriate resources.
  • Follows HIPAA guidelines at all times.
  • Assists in development and revision of policies and procedures
  • Attends organizational meetings and training as required.
  • Keeps Director and Lead informed of Intake and Assessment problems and progress.
  • Meets productivity goals of the department.
  • Refers clients on the waiting list to other community resources for assistance.
  • Maintains standard of promptness for all programs.
  • Completes all appropriate documentation/paperwork for all programs.
  • Completes full rescreens for clients on CCSP and HCBS waiting list when required.
  • Provides information and assistance to callers as well as options counseling as needed with follow up as required.
REQUIREMENTS EDUCATION, EXPERIENCE AND SKILL REQUIREMENTS

Education:
  • Bachelor’s degree in Social Work, Sociology, Psychology, or a related field or a Registered Professional Nurse currently licensed to practice in the State of Georgia.
Knowledge and

Experience:
  • Two years’ experience in the human service or health related field plus one year of experience as a case manager.
Skills:
  • Computer skill proficient enough to master the Wellsky data base system
  • Well organized, detail oriented and able to work independently and/or as part a team
  • Ability to communicate with clients having a variety of educational levels.
Licensure and Certification
  • Valid State Driver’s License
  • Will be eligible to take the CIRS-A/D exam after one year of experience
  • Will be eligible to take Boston university Courses after becoming CIRS-A/D certified and one year of experience.
