Independent Contractor-Family Support Services

Health Connect America, Inc. seeks an Independent Contractor for Family Support Services to provide in home and community based support to children, adolescents, and families. In this role, you will deliver skill building, parent coaching, behavior support, and case coordination under clinical guidance, helping stabilize placements and improve family functioning. You will collaborate with therapists, schools, and community partners; document services;

and maintain timely communication. Ideal for mission driven professionals passionate about accessible behavioral health and strengthening families in their local communities.

Responsibilities

• Provide in-home and community-based family support and skill-building services
• Implement treatment and service plans under clinical or supervisory guidance
• Offer parent coaching, behavior support, and advocacy for children and families
• Coordinate with therapists, schools, and community agencies to support client goals
• Monitor family safety and stability, identifying and reporting concerns as required
• Maintain accurate, timely documentation of services and progress notes
• Participate in team meetings and case staffings as requested
• Manage schedule and travel to deliver services in homes and community settings
