Pay: $33,800 per year

Program:
Permanent Supportive Housing/Emergency Solutions Grant

This position may serve the following counties:
Harris, Talbot, Randolph, Clay, Stewart, Quitman, and Chattahoochee.

Works with the following population:
Males, females, and families experiencing mental illness, substance abuse disabilities, and/or homelessness.

Hours & Days:
Monday through Friday (8am to 5pm). 40 hours per week.

Job Responsibilities:

The Rural Housing Outreach Case Manager will perform outreach services in the community and will work with individuals experiencing homelessness, mental health and substance abuse disabilities. The candidate will also:

  • Provide case management to help families access resources and help link them to services needed, while monitoring progress.
  • Identify needs and provide resources for local law enforcement, churches, family connection coordinators, and other community partners.
  • Educate communities on Opioids and Narcan use along with distribution of Narcan.
  • Develop good working relationships with government and community resources.
  • Maintain client-related data tracking systems and document all services provided in electronic medical records.
  • Transport individuals to appropriate services, as needed.
  • Develop rapport with individuals; encourage and support client participation in relevant activities and events.
  • Provide advocacy for individuals to minimize barriers.
  • Assist with planning and implementation of services that meets the clients' needs.
  • Identify appropriate permanent housing options for clients experiencing homelessness.
  • Other duties as assigned by leadership.
Minimum Qualifications:

High school diploma or GED. At least two (2) years of experience working with individuals with mental illness, substance abuse, and/or homelessness.
Must have a valid driver's license, a personal insured vehicle, and a clean driving record, as transporting is required.

Preferred Qualifications:

A bachelor's degree in the field of Human Services. 3+ years of experience working with the homeless population. Certified Peer Specialist.
