Social Services Coord

As a Social Services Coordinator on our team, you will contribute to supporting residents’ psychosocial well-being, assisting with care planning, and connecting residents and families with the resources they need to thrive within our community.

• Review potential residents for admission and provide required PAS information in compliance with state regulations.
• Develop initial care plans based on residents’ physical and psychological needs.
• Continuously update resident care plans and documentation regarding resident condition.
• Provide direct counseling to residents, families, and resident representatives.
• Collaborate with the healthcare team to identify and address psychosocial, spiritual, and mental health needs.
• Assist residents in adjusting to the community and promote continuity of family and community relationships.
• Coordinate relocation planning and facilitate transitions within levels of care.
• Safeguard confidential information related to resident social histories and interviews.
• Participate in the quality assurance process as assigned.
• Perform MDS assessments and support the RAI process as needed.

M-F 8:00 A – 5:00 P Typically, but can change based on the needs of residents

• High school diploma or GED equivalent.
• Successful completion of the 20-hour social service course or other approved social worker designee program within the state.
• Strong understanding of social systems, community resources, and support programs.
• Effective communicator with the ability to provide compassionate counseling and advocacy.
