What you need to know about this position:
- A member of the Columbus Regional Health Physicians team, OB/GYN Associates of Columbus, is a fast-paced practice with 8 providers and averages 80 patients a day.
- OB/GYN Associates of Columbus treats women throughout all stages of their life.
- The Nurse Navigator coordinates and manages care to meet multiple needs of clients, ensures continuity of services, provides emotional support and patient education and overcomes fragmentation of care and services.
- The main goals of the Nurse Navigator are to guide and support the patient and his/her family throughout the duration of their time in the program.
- The Nurse Navigator develops long-term relationships with patients and coordinates care in all settings across the continuum.
- The Navigator promotes an open system of care in which information is shared among all disciplines working collaboratively on behalf of the patient and family in the mutual planning for care in the Center, the hospital, and home.
- The navigator acts on behalf of the patient in assuring that needed interventions are obtained and the patient is making progress according to mutually defined goals.
- The Navigator is responsible and accountable for appraising the specific as well as the overall effectiveness of the Unit based case management nursing services.
- The base hourly rate for this position is $35.82. Individual compensation is determined for this position through years of directly relevant experience. The hourly compensation is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.
- This position will require you to work 8:00am-5:00pm, Monday through Friday, approximately 28 hours a week.
Education and/or Experience
- Graduate of an accredited program in professional nursing.
- RN required; currently licensed to practice nursing in Indiana. Graduate of approved School of Nursing
- Basic Life Support (BLS) certification required.
