Position Overview

A growing employer-sponsored outpatient healthcare program is seeking an experienced Nurse Practitioner or Physician Assistant to join the team as a PRN/Float Provider
. This position provides coverage across multiple clinic locations throughout Indiana, including Columbus, Anderson, West Lafayette, Hammond, and Fort Wayne
. This is an excellent opportunity for an experienced provider who enjoys practicing independently in a patient-focused primary care environment while supporting preventive health, wellness, and occupational healthcare initiatives. In addition to hourly compensation,
mileage reimbursement is provided for travel between assigned clinic locations.

Schedule
  • PRN/Float schedule based on clinic coverage needs
  • Travel required between assigned Indiana clinic locations
  • Mileage reimbursement provided
Key Responsibilities
  • Perform pre-employment physicals and annual wellness examinations
  • Evaluate, diagnose, and manage acute and chronic medical conditions using evidence-based clinical practices
  • Develop and implement individualized treatment plans within supervising physician protocols, when applicable
  • Assess and treat workplace injuries and common illnesses
  • Deliver preventive care services, health screenings, and wellness programs
  • Educate patients on healthy lifestyle choices and disease prevention
  • Support weight management through nutrition, exercise, behavioral coaching, and medication management, including GLP-1 therapies when appropriate
  • Order, review, and interpret diagnostic tests
  • Monitor patient progress and adjust treatment plans as needed
  • Coordinate referrals to specialists and other healthcare providers
  • Maintain accurate and timely documentation in the electronic medical record
  • Collaborate with clinic staff to deliver high-quality patient care
  • Assist with additional clinical and operational responsibilities as needed
Required Qualifications
  • Board Certified Nurse Practitioner or Physician Assistant with an active Indiana license (or ability to obtain)
  • Minimum of 1 year of experience in Family Medicine, Internal Medicine, Urgent Care, or another primary care setting
  • Comfortable practicing independently in a clinic environment
  • Strong clinical assessment and decision-making abilities
  • Current BLS Certification
  • Excellent interpersonal and patient communication skills
  • Commitment to preventive, patient-centered healthcare
  • Willingness and ability to travel between assigned Indiana clinic locations
Preferred Qualifications
  • DEA Registration
  • Experience in employer-sponsored health clinics, occupational medicine, or wellness programs
  • Experience managing patients participating in weight management programs
  • Familiarity with obesity treatment medications, including GLP-1 therapies
  • Experience working with multidisciplinary healthcare teams
