This role works with organizational management to implement and maintain a nationally recognized Diabetes Self‑Management Education and Support (DSMES) program for the health system. The position performs assessments of diabetic patients’ physical health, medications and psychosocial issues, identifying educational and clinical intervention areas based on current national diabetes care standards and guidelines. The individual collaborates with patients, medical provider teams and other care specialists to develop and implement diabetes care plans, maintains medical records, provides education on self‑care practices, and serves as a diabetes care resource to healthcare professionals and the community.

Education
  • Required – Registered nurse diploma or bachelor’s degree in a related field
  • Preferred – Master’s degree in a related field
Work Experience
  • Required – 3 years of experience with direct patient contact
Certifications
  • Preferred – Certification as a Diabetes Care and Education Specialist (CDCES) by the Certification Board for Diabetes Care and Education Specialists (CBDCES)

  • Ability to adapt to changes in systems, practices, technology and fluctuating variables in a practice setting such as patient volume, acuity, skill, experience and training of personnel, utilization of supplies and equipment, and manpower resources
Job Duties
  • Guide persons with diabetes in establishing individualized behavioral and clinical goals, develop success parameters and use behavior change methodology to facilitate patient participation in the education process
  • Develop an education plan to address behavioral goals, create a learning plan to address knowledge gaps, collaborate with patients to plan strategies for addressing barriers, and refer to the prescriber as needed
  • Consult and coordinate with health‑care team members to assess, plan, implement and evaluate patient care plans to enhance satisfaction, care and safety
  • Instruct individuals, families and other groups on diabetes education, disease prevention and management, and develop health improvement programs
  • Recommend pharmacological treatment options, medical devices or other forms of treatment as needed, such as insulin pumps, continuous glucose monitors or temporary continuous glucose monitors, and alternative forms of treatment due to financial, ambulatory or disability needs
  • Adapt behavior to the specific patient population, including respecting privacy, explaining services or procedures, requesting permissions and employing appropriate communication style
  • Perform other related duties as assigned
