What You'll Do- Conduct comprehensive evaluations and create personalized treatment plans tailored to each resident's goals
- Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and overall function
- Monitor progress, celebrate achievements, and adjust treatment plans to maximize results
- Collaborate closely with nursing staff, physicians, and interdisciplinary team members to provide holistic, high-quality care
- Educate residents and families on treatment strategies, recovery expectations, and wellness goals
- Maintain timely and accurate clinical documentation in accordance with regulatory standards
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist (PT) license in good standing
- Experience in skilled nursing, rehabilitation, or long-term care preferred
- Excellent communication, interpersonal, and problem-solving skills
