Position Overview
A value-based care organization is establishing a new outpatient practice in central Nebraska and is adding a Board-Certified Family or Adult Nurse Practitioner to its care team. The role is based in a newly opened facility and offers the opportunity to help shape clinical operations as one of the first providers in the region.
Responsibilities- Provide telehealth, home visits, and outpatient clinic care focusing on longitudinal care for patients with complex medical needs.
- Help shape clinical operations as one of the first providers in the region.
- Board-Certified Family or Adult Nurse Practitioner.
- Experience in Geriatrics, Home Care, or house-call setting preferred but not required.
