Job Title: RN Case Manager – Floating Territories | Home Health, 1:1 Care + Incentive Pay

Compensation: $95,000 – $130,000 per year plus incentive visit pay

Location: Greater Twin Cities Metro Area (21‑county service area)

Schedule: Full‑Time with On‑Call Rotation

• Step into different territories to support caseload coverage and census needs
  .
• Provide skilled nursing care, assessments, and care planning
  .
• Complete OASIS visits and coordinate interdisciplinary care
  .
• Educate patients and families on care plans and recovery goals
  .
• Collaborate with physicians, therapists, and care teams.
• Travel throughout the metro while maintaining flexibility and independence
  .

• Strong clinical judgment and ability to adapt quickly.
• Comfortable working independently in a field‑based, travel‑focused role
  .
• Experience in home health preferred, but we will train the right RN.
• Reliable transportation and willingness to drive throughout the metro.
