Scope/Position SummaryThis position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas. Duties include but are not limited to: assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses. Patient population includes adult, adolescent, and geriatric patients.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising pharmacy residents and students, medical residents and students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy).
Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.
D./MS) and residency; and/or equivalent experience preferred. Ability to perform point-of-care INR.
