Hospice Caregiver

Are you a compassionate caregiver with a heart for supporting clients and families during their most meaningful moments? Synergy Home Care is looking for dedicated Hospice Caregivers to provide comfort‑focused, in‑home support to clients receiving end‑of‑life care.

As a Hospice Caregiver with Synergy Home Care, you will provide essential non‑medical support that helps clients remain comfortable and safe in their own homes. You’ll work closely with families and hospice teams to ensure emotional support, dignity, and comfort are always at the center of care.

• Provide compassionate companionship and emotional support
• Assist with personal care (bathing, dressing, grooming, toileting)
• Offer comfort‑focused care and a calm, reassuring presence
• Support mobility and positioning
• Prepare light meals and assist with feeding as needed
• Perform light housekeeping to maintain a safe environment
• Follow care plans in collaboration with hospice nurses and families
• Document care notes accurately and promptly
• Respect client dignity, cultural needs, and family wishes

• Experience in hospice, palliative care, or caregiving preferred
• A calm, patient, and empathetic personality
• Ability to provide emotional support during sensitive situations
• Reliable, punctual, and professional
• Valid driver’s license and reliable transportation
• Ability to pass background check and meet onboarding requirements
