Unit Specific Information
- Iowa - 1860 mask only.
- Positive TB requires a chest xray within the last year for Michigan, Indiana, Connecticut and Massachusetts. All other regions within 5 years.
- Resume, Skills Checklist within a year, 2 Professional References required.
- Not required for non-clinical. For clinical contractors, the agency will provide this checklist.
- 10-panel drug screen completed within 30 days of start.
- All engaged contractors must have page 27 signed form on file in Trinity VMS.
- Standard i9 completed by the agency.
- Trinity Health Confidentiality of Patient and Proprietary Information and Information System Security Agreement required.
- Signed within days prior to start.
- Attach copy of specialty exam must be completed within a year.
- Proof of vaccine Sept 2025 booster, declination or exemption (with agency letterhead). Declination not accepted for CA or IL. End date of October 2026.
- Attached scanned proof of 2 vaccinations with dates or primary source IgG titer with lab print-out.
- Must have 1st vaccination in series prior to start; 2nd shot in series 30 days later; 3rd shot in series 6 months later. Hep B titers will NOT be accepted.
- Proof of vaccine, declination or exemption (with agency letterhead). Declination not accepted for CA or IL. Flu dates: 10/1/2025-4/30/2026.
- Attached scanned proof of 1 vaccination with date or primary source IgG titer with lab print-out.
- Provide documentation from an exam within the last year that candidate is fit to work.
- All engaged contractors must have page 1 signed form on file in Trinity VMS.
- Federal, Nationwide, County Criminal from last 7 years. Also include SSN Verification. Must be dated within 30 days of start.
- Attach completed 3rd Party Screening Company report of candidate's highest level of academic education.
- Attach scanned EPLS report from website or result provided by third party screening company dated within past 30 days.
- Attach scanned OIG result print-out from the website or result provided by third party screening company and dated within past 30 days.
- CORE Exam within a year.
- Must be dated within 30 days of start. Check all names used and locations. Attach scanned report to candidate's record in Trinity VMS.
- Must be completed and uploaded one week before start.
