Role Description

This is a full-time on-site role based in Columbus, OH for a Palliative Care Social Worker. The role involves providing emotional and psychosocial support to patients and their families in palliative care settings. Key responsibilities include conducting patient assessments, developing individualized care plans, offering counseling services, and collaborating with interdisciplinary teams to ensure comprehensive care. The Social Worker will also help patients and their families navigate available resources, advocate for patient needs, and support end-of-life care planning and decisions.

Qualifications

  • Strong skills in case management, counseling, and assessment of emotional and psychosocial needs
  • Knowledge of interdisciplinary teamwork and experience in working collaboratively with healthcare providers
  • Understanding of palliative care, grief and bereavement counseling, and end-of-life care processes
  • Effective communication and active listening skills to provide support and advocacy for patients and families
  • Proficiency in navigating community resources and connecting patients with appropriate services
  • Master's degree in Social Work (MSW) or related field required; licensure as an LSW or LISW in the state of Ohio preferred
  • Experience working in palliative care, hospice, or healthcare settings is highly desirable
