Summary:

*** This is a full-time, hybrid position in the Mansfield region. This role supports a newly approved expansion with defined implementation timelines, so prior experience is strongly preferred. We will prioritize candidates that can begin in early to mid-spring to support program launch and outcome tracking.**
* JOB SUMMARY

Ambulatory Care Pharmacist – Population Health Pharmacy Services

This position is responsible for improving disease management in our Ohio Health Physician Group – Primary Care, CIN, and Direct to Employer programs. The pharmacist will collaborate with primary care providers to provide disease state management programs to primary care patients; currently working under a consult agreement for the management of patients with diabetes and hypertension. This position supports HEDIS quality metrics and care gap closure while also delivering asthma and diabetes management services to our employee sponsored health and wellness patients.

As part of a multidisciplinary team, this role supports a hub of Ohio Health Physician Group primary care offices in a hybrid role with 2-3 days in the primary care offices and a large portion of patient care occurring via telehealth.

MINIMUM QUALIFICATIONS AND SPECIALIZED KNOWLEDGE

• Bachelor of Science or Doctor of Pharmacy degree from an accredited institution (Required)

• Current Ohio pharmacist license (Required)

• Disease state management, especially diabetes. Ideally have experience managing referrals, schedules and working under a collaborative practice agreement.

• Population Health perspective, including HEDIS Quality Metrics & care gap closure experience.

• Basic Life Support Certification—current or obtained during the orientation
period (unless greater than 50% of scheduled work hours, as determined by
the manager, are completed off-site or not in a patient care setting)

• ASHP accredited residency or equivalent pharmacy practice experience

• Pharmacy Board Certifications or other certifications

• Advanced-level degree in healthcare field

• Collaborative practice experience

• Advanced Cardiac Life Support Certification (may be required to obtain as
determined by manager)

• Pharmacokinetic drug dosing experience

• Drug therapy management

Responsibilities And Duties:

Acute Care/Infusion

50% - Reviews provider orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts providers to clarify orders as appropriate. Dispenses and compounds medications and dispenses drug therapy-related devices in accordance with standards of practice and Ohio Health pharmacy policies. Ensures all medications, chemicals and supplies are dispensed without suboptimal integrity. Ensures appropriate labeling for proper administration of medication.

Interprets prescriptions and determines compatibility in accordance with standards of practice. Prepares care plan for patients to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.

20% - Manages individuals’ drug therapy by initiating, monitoring, and modifying prescriptions issued to patients in a consulting relationship with a provider. Provides drug information to providers, other health care professionals, and patients as needed.
