Job Description SummaryThis position ensures organizational excellence related to customer service and quality of care while promoting excellence in the cultural experience at the assigned hospital. The Social Worker (MSW, LISW) provides services to patients, families and significant others, including biopsychosocial assessments, psychiatric diagnoses, formulation of treatment plans and interventions, therapy sessions (individual, couple, family, or group), implementation of treatment plans, and discharge planning to support and enhance biopsychosocial functioning.
The role includes advanced functions such as staff education, development, training, mentoring, project development, and administrative or supervisory responsibilities that advance the department and organizational mission. The clinician may practice independently and without supervision as permitted by Ohio licensure, and may supervise select social service functions, oversee scheduling, and assist the manager with direct supervision of social work staff and master’s level interns and students.
Responsibilitiesand Duties
- 30% Advanced Clinical Assessment
- Conduct patient/family/significant other interviews, chart reviews, and professional collaboration to obtain and interpret information on biopsychosocial functioning.
- Develop care plans, including discharge planning, incorporating medical plans of care.
- Identify cases of abuse, neglect, or exploitation and refer to legally mandated authorities.
- Provide crisis intervention, information, referral, and linkage to community resources as needed.
- 30% Advanced Intervention Management
- Develop intervention plans based on assessment to resolve identified problems or support biopsychosocial functioning.
- Employ individual, family, or group modalities, including treatment planning, continuum of care planning, supportive counseling, crisis intervention, specialized problem-oriented planning, and educational counseling.
- Collaborate with interdisciplinary staff for holistic care planning.
- 25% Advanced Population Management
- Coordinate clinical and financial outcomes for assigned populations.
- Collaborate with interdisciplinary staff and external organizations to ensure timely, cost‑effective management of patient care.
- Monitor quality of services, manage post‑discharge financial resources, and ensure complete treatment plan reporting.
- Facilitate care coordination and access to resources for families.
- 10% Documentation and Regulatory Compliance
- Thoroughly document assessments and interventions in the patient’s medical record and information systems per department standards.
- Comply with internal and external documentation requirements and regulatory obligations.
- 5% Ongoing Awareness and Linkage
- Maintain awareness of current hospital, community services, and appropriate care options.
- Provide information and facilitate successful linkages between services and the patient/client.
• Master’s Degree in Social Work (required)
• LISW – Licensed Independent Social Worker – Social Work Certification and Licensure Board
• LPCC – Licensed Professional Clinical Counselor – American Counseling Association
• 1–2 years Social Work experience in health care after Master’s Degree and LISW license.
• Experience in Behavioral Health with acutely ill psychiatric patients.
• Medical terminology knowledge and understanding of disease processes.
• Leadership competencies, including staff education, training, and mentorship.
• Information security: maintain confidentiality, adhere to privacy policies, report incidents.
• Regularly required to use hands, talk, hear; occasionally lift up to 25 pounds.
