About Solero Behavioral TransitionsSolero Behavioral Transitions offers a safe, structured environment for adults managing substance use and mental health challenges. As a dual diagnosis outpatient program, we provide integrated treatment for individuals with primary substance use or mental health disorders.
Responsibilities include:- Provide direct patient care during overnight hours, including assisting patients with activities of daily living (ADLs) as needed while promoting dignity, independence, and safety.
- Monitor patients throughout the night for behavioral, psychiatric, medical, and withdrawal-related changes, promptly reporting any concerns or significant observations to the Registered Nurse (RN).
- Perform scheduled and as-needed (PRN) clinical tasks, including obtaining vital signs, blood glucose monitoring, withdrawal assessments, and other delegated patient care activities within scope of practice.
- Conduct routine safety rounds, continuous observations, contraband searches, and monitor unit access to maintain a safe and secure therapeutic environment during overnight hours.
- Assist with overnight admissions, patient orientation, room preparation, and early-morning discharge activities to ensure a smooth transition of care.
- Facilitate and document approved overnight therapeutic, recreational, or recovery-focused activities as scheduled, while providing individual patient support and encouragement.
- Respond promptly and professionally to behavioral health crises by utilizing de-escalation techniques, crisis intervention skills, and established safety protocols.
- Maintain a clean, quiet, and therapeutic environment that promotes patient rest, healing, and recovery in accordance with hospital policies and procedures.
- Complete accurate and timely documentation of patient observations, interventions, and assigned responsibilities in the electronic medical record.
- Communicate effectively with patients, Registered Nurses, providers, and members of the interdisciplinary treatment team to ensure continuity of care between shifts.
- Participate in shift report and handoff communication to provide accurate updates on patient status, safety concerns, and ongoing care needs.
- Adhere to all hospital policies, infection prevention standards, patient confidentiality requirements, and regulatory guidelines while delivering high-quality patient care.
- High school diploma or GED.
- Must be 21 or older
- Associates or Bachelor's Degree in Psychology, Sociology, or Behavioral Health field is a plus.
- Demonstration of group facilitation skills, knowledge of psychiatric diagnosis and treatments.
- Crisis intervention skills. Handle with Care training/certification preferred, but will train.
