Job Overview – Clinical Pharmacist (Home Infusion)
Compensation: $125,000 – $135,000/year

Schedule:
Monday to Friday, 8:30 AM – 5:00 PM
TAG Med Staffing is hiring a Clinical Pharmacist (Home Infusion) in Columbus, OH for our client, supporting home infusion pharmacy services, sterile compounding, medication therapy management, and patient care. This full-time opportunity is ideal for a licensed pharmacist with experience in acute care or home infusion who is passionate about delivering high-quality clinical pharmacy services in an alternate care setting. The role combines direct patient care, clinical oversight, and collaboration with multidisciplinary healthcare teams.
Responsibilitiesas the Clinical Pharmacist (Home Infusion):
- Clinical Pharmacy Services: Assess prescribed therapies, review patient clinical information, develop individualized medication plans, and provide ongoing pharmacotherapy management.
- Medication Preparation: Oversee sterile compounding, prescription verification, intravenous admixture preparation, and medication dispensing while ensuring compliance with USP and regulatory standards.
- Patient Care Coordination: Collaborate with physicians, nurses, dietitians, and caregivers to optimize therapy, monitor patient progress, and support continuity of care.
- Clinical Documentation: Maintain accurate medication profiles, clinical records, laboratory monitoring, therapy documentation, and patient education records.
- Pharmacy Operations: Supervise pharmacy support staff, support workflow efficiency, assist with quality assurance activities, and ensure compliance with pharmacy policies and procedures.
- Professional Support: Provide drug information, participate in patient care conferences, contribute to staff training, and assist with on-call responsibilities as needed.
- Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited college of pharmacy is required.
- Licensure: Current pharmacist license in the state of employment is required.
- Experience: Minimum of 2 years of recent acute care pharmacy experience or 1 year of recent home infusion pharmacy experience is required.
- Industry Knowledge: Strong knowledge of intravenous admixture services, sterile compounding, pharmacokinetics, infusion therapy, and home infusion pharmacy practices is required.
- Technical
Skills:
Proficiency with pharmacy management systems, electronic medical records, Microsoft Office, and pharmaceutical calculations is required. - Skills & Attributes: Strong clinical judgment, communication, organizational, problem-solving, and leadership skills with the ability to supervise support staff, manage multiple priorities, and collaborate effectively within a multidisciplinary healthcare team.
