Licensed Physical Therapy Assistant

Ohio Living Home Health -  - Responsibilities:
Implements the Plan of Care developed by the Physical Therapist, ensuring treatment per physician's orders;
Communicates with the Physical Therapist, physician, and staff about physical/medical concerns pertaining to the Plan of Care;
Educates patients and family members on exercise techniques and self-care activities to ensure safety and independence;
Performs home visits to implement the Plan of Care and improve function and safety at home;
Maintains patient records through electronic medical record documentation
