The Patient & Provider Advocate is an operationally critical role in And Health's Provider partnership model. Every successful partnership we have built has involved someone who owns patient identification and engagement at the practice level.

The Patient & Provider Advocate is forward-deployed — present in partner practices and at times working directly in their EHR. They identify eligible patients, queue them for provider review, navigate access challenges, and ensure that the white-glove service And Health has promised is actually felt by both the partnered practice and the patient.

What you’ll do in the role:
Patient identification and enrollment
  • Manage patient enrollment process from referral receipt through successful program onboarding
  • Work within partner practice (including in EHR systems) to identify CDM-eligible patients in target specialty (MS, derm, rheumatology)
  • Queue identified patients for provider review and referral — removing the burden from practice staff entirely
  • Conduct outbound patient contact as needed to support enrollment and transition — warm, professional, and brand-appropriate
  • Track enrollment funnel for each practice account and surface patterns that indicate process gaps, with solutions
  • Serve as the on-site expert when a physician or staff member encounters a medication access challenge
Practice support and relationship
  • Build working relationships with practice coordinators, medical assistants, and office managers — the people who execute the day-to-day
  • Provide white-glove service for referrals from Advocates— VIP flagging, dedicated care coordination, and enhanced status communication back to the practice
  • Train and remind practice staff on referral workflows — proactively, not reactively
  • Identify and surface practice-level issues to the Provider Partner Account Management Director and Strategic Account Director before they affect the relationship
Documentation and reporting
  • Maintain accurate patient-level records in And Health's systems for all assigned accounts
  • Provide regular status updates to Provider Partner Account Management Director and Strategic Account Director on enrollment activity, access barriers, and practice dynamics

    Partner with Account Management to document any bespoke workflows or commitments unique to each practice for knowledge continuity
Education & Licensure Requirements:
  • Bachelor’s Degree required.
Other Skills or

Qualifications:
  • 5+ years in a field reimbursement, patient access, or clinical operations role
  • While working collaboratively and connecting dots internally, comfort working independently and autonomously while in provider offices — you are a self-directed professional who doesn't need daily supervision
  • Strong interpersonal skills with clinical and administrative staff at all levels
  • Experience supporting specialty patient populations (neurology, dermatology, rheumatology, or other autoimmune specialties) a plus
  • Willingness to travel regularly; expected to be on-site at partner practices most days per week
  • Familiarity and willingness to use AI tools.
