Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A specialized clinical pharmacist role is available for a direct hire position focused on home infusion therapy in Columbus, OH. This opportunity is ideal for pharmacists with IV certification and experience in complex intravenous medication preparation and patient-centered care within a home infusion setting.

Schedule & Hours
  • Monday to Friday, 8:30 a.m. to 5:00 p.m.
  • Participate in on-call rotation
Key Responsibilities
  • Prepare sterile IV and specialty pharmaceuticals in compliance with all state and federal sterile compounding regulations.
  • Monitor clinical parameters including lab values and pharmacokinetic dosing (e.g., vancomycin) and adjust Total Parenteral Nutrition (TPN) as needed.
  • Educate patients and caregivers on infusion pump use, sterile techniques, and catheter care to ensure safe administration at home.
  • Collaborate with physicians, home health nurses, and interdisciplinary teams to coordinate therapy and minimize hospital readmissions.
  • Maintain regulatory compliance with the State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant regulatory standards.
  • Manage pharmacy inventory, including temperature-sensitive products, and coordinate shipping of medications to patient homes.
Qualifications & Desired Experience
  • Minimum 2 years of infusion specialty pharmacy experience.
  • IVIg experience is an advantage.
  • Well-versed in specialty therapy payer requirements, reimbursement processes, and contract compliance.
  • Current and active Ohio pharmacist license in good standing.
  • Experience with the Care Tend system preferred.
