Clinical Pharmacist
Equity Staffing Group is seeking a Clinical Pharmacist to join a Clinical Engagement Services Center supporting programs including Medication Therapy Management (MTM), medication adherence, diabetes, polypharmacy, retrospective drug utilization, STARS adherence, and Fraud, Waste & Abuse initiatives.
This role is primarily responsible for conducting telephonic consultations with members
, completing comprehensive medication reviews, addressing medication adherence, and helping improve disease-state management.
Monday-Friday | Flexible rotating shifts between 9:00 AM-8:00 PM CST
Extended hours and occasional weekends may be required.
Key Responsibilities- Conduct telephonic consultations and comprehensive medication reviews with members.
- Schedule pharmacist consultation appointments for members participating in clinical programs.
- Review prescription claims and clinical information to identify gaps in care, high-risk medications, adherence concerns, drug interactions, therapeutic substitutions, and generic alternatives.
- Assess medication regimens for unnecessary or ineffective medications, inappropriate dosing, and potential adverse drug reactions.
- Provide patient education regarding medications and disease-state management.
- Communicate clinical recommendations and relevant information to members' healthcare providers.
- Accurately and efficiently document patient care, interventions, and recommendations.
- Maintain HIPAA compliance and protect patient confidentiality.
- Meet established productivity and performance expectations.
- Participate in departmental initiatives, process improvements, and special projects.
- Current and unrestricted Pharmacist license in state of residence
. - Strong knowledge of disease-state management and the 5 Core Elements of MTM Services
. - 1+ year of experience working remotely/independently within a cross-functional team environment.
- Ability to navigate MTM platforms and work across multiple databases and applications simultaneously.
- Intermediate proficiency with Microsoft Word, Outlook, and Excel
. - Strong organizational, documentation, communication, and time-management skills.
- Ability to work flexible rotating shifts within the required schedule.
- PharmD
. - 2+ years of pharmacy experience in retail/community pharmacy and/or PBM; new graduate experiential experience may be considered.
- Previous call center experience
. - Diabetes-related certification such as CDCES, BC-ADM, ASHP Diabetes Management Certificate, or APhA Patient-Centered Diabetes Care Certificate.
- Bilingual in Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese
, including medical terminology. - Additional active Pharmacist licenses in AL, , KS, KY, LA, MD, MN, MO, ND, OK, OR, TN, VT, or WV
.

Successful candidates will have strong clinical judgment, active listening and problem-solving skills, excellent attention to detail, and the ability to manage multiple priorities in a fast-paced environment.
