The Acute Crisis Coordinator is responsible for processing high-acuity referrals for the youth population experiencing a behavioral health crisis requiring rapid access to services. This role supports referrals from child-serving agencies, hospitals, and crisis services. This position plays a vital role in building and maintaining collaborative relationships across organizations, facilitating timely service access, removing barriers to care, and supporting positive outcomes for youth and families during periods of acute behavioral health need.
It will also require strong organizational skills, attention to detail, professionalism, and the ability to navigate multiple software systems in a fast-paced work environment.
The pay range for this position is $21.09-$25.03 per hour based on experience, education, and/or licensure. This role is 60% in-office, 40% virtual.
Essential Functions:Receive and process acute behavioral health referrals accurately and efficiently in the electronic health record (EHR) from various inpatient units, emergency departments, crisis response teams, schools, child welfare agencies, and juvenile justice partners
Perform detailed and timely data entry, including child demographics, guardian information, presenting concerns, referral source details, crisis history, and payer information
Answer the acute referral phone line & Microsoft Teams Channel, return urgent messages promptly, and document all interactions accurately
Coordinate the rapid review and assignment of high-acuity referrals by partnering closely with internal licensed social workers to ensure timely triage, clinical assessment, and service recommendations.
Ensure continuity of care through proactive communication and collaboration with referral sources, clinical staff, and community partners to facilitate rapid access to appropriate services.
Utilize databases and Electronic Health Record systems for accurate client documentation???
Adhere to organizational EHR documentation policy??
Adhere to time parameters related to compliance requirements
Meet the organization's standards for productivity and quality of service delivery
Maintain compliance with HIPAA, mandated reporting requirements, and professional ethical guidelines?
Perform other duties as assigned
Education/Licensure:A high school diploma or GED equivalent is required
An associate degree or bachelor's degree/postsecondary certificate in healthcare, behavioral health, human services, or a related field is preferred
Experience:3 years of experience working in a medical referral position and/or nonprofit administrative support function or related field is required
Experience working with children, adolescents, and families with acute behavioral health needs and/or crisis is preferred
Knowledge, Skills and Abilities:Advanced verbal and written communication skills are required
Ability to manage competing priorities is required
Proficiency in electronic health records, databases, and related software is required
Ability to manage deadlines within a fast-paced, high-volume environment is required?
Ability to work collaboratively with multidisciplinary clinical and community teams
Ability to operate in a hybrid, internet-based work environment
A valid driver's license and the ability to operate a motor vehicle is required?
An appropriate level of auto insurance coverage is required?
Knowledge of pediatric behavioral health systems, crisis response, and levels of care is preferred
Knowledge of insurance verification, Medicaid billing, and authorization processes is preferred
