Outreach Specialist

Bold Age PACE seeks an Outreach Specialist to connect older adults and their families with our comprehensive care program. In this role, you will build relationships with community partners, clinics, and senior organizations to identify eligible participants and explain program benefits. You'll conduct presentations, attend health fairs, and manage follow-up with prospective enrollees, guiding them through the application and onboarding process.

Working in a collaborative, compassionate environment, you'll partner closely with clinical and social work teams, using your communication and organizational skills to expand access to coordinated care for seniors.

Responsibilities

• Conduct outreach to eligible seniors and caregivers to promote Bold
• Age PACE services
• Build and maintain relationships with community partners, clinics, and senior organizations
• Educate prospective participants and families about program benefits and enrollment process
• Coordinate and attend community events, health fairs, and presentations
• Track outreach activities, leads, and conversion metrics in CRM or database
• Collaborate with clinical and social work teams to support smooth participant onboarding
• Assist participants with application paperwork and documentation collection
• Provide culturally sensitive communication tailored to diverse older adult populations
• Develop outreach materials and contribute to marketing campaigns targeting seniors
• Report outreach outcomes and community trends to leadership for program improvement
