We’re looking for an organized and patient‑focused LPN to join our Pulmonary and Sleep Medicine clinic in Reynoldsburg, OH. In this role, you will be able to make a meaningful difference for patients managing sleep apnea and chronic lung conditions. You’ll be part of a small, supportive team that values collaboration, work‑life balance, and a calm yet engaging clinical environment.
In this role, you’ll support the daily operations of the sleep and pulmonary clinic by rooming patients, managing MyChart messages and phone calls, and coordinating care between visits. Responsibilities include scheduling and following up on sleep studies, assisting with CPAP evaluations, obtaining vitals, and helping patients with results and equipment orders. You’ll also handle administrative and triage tasks balancing both patient‑facing and backend duties.
Travel may be needed to nearby Ohio Health sites to assist with coverage. This is a great opportunity for nurses who enjoy variety, direct patient interaction, and a strong sense of teamwork.

This position provides nursing care to patients and families along the health illness continuum in diverse health care settings while collaborating with the health care team. The LPN is accountable for the practice of nursing as defined by the Ohio Board of Nursing. The LPN delegates nursing tasks to unlicensed assistive personnel (UAPs) using the criteria written in the Ohio Nurse Practice Act.
This position serves as a resource for other associates. May communicate instructions, do minor scheduling, and inspect work. Does not hire, fire, discipline, promote, demote, transfer, or appraise, but may be consulted by higher management in these areas. This job has no supervisory responsibilities.
25% Performs initial, ongoing and functional health status assessment as applicable to the population and/or individual. Collects and documents initial and ongoing subjective and objective data from patients, significant others, and communities. Reviews data obtained by other members of health care team and incorporates into assessment. Identifies and communicates assessment findings, both initial and ongoing, to appropriate people via referrals, rounds, huddles, reports and care conference, etc.
Uses assessment data to identify pertinent nursing diagnoses and collaborative problems. Collaborates assessment findings with providers and members of healthcare team as appropriate. Uses scientific knowledge, Critical thinking skills, technical skills, and effective interpersonal and teaching skills to provide nursing care.
25% Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs including: age, size, and development stage. Initiates and/or coordinates care plans and individualizes critical paths/medical management plans. Promotes cost effective care by minimizing fragmentation, maximizing coordination and facilitating patient/family through the health care system. Accountable for quality improvement and clinical outcomes and any other duties assigned.
