About the role

We are seeking a licensed Ohio Registered Nurse to serve in a centralized Internal Triage role supporting all Ohio markets during after-hours coverage. This position functions as the clinical coordination hub during evenings and weekends, ensuring patients and families receive timely guidance, appropriate clinical direction, and seamless care transitions.

This is a fully remote role with no routine travel required. The ideal candidate is confident in hospice triage, exercises strong clinical judgment, and is comfortable independently managing both patient needs and operational coordination during after-hours periods.

Job Responsibilities
  • Answer and triage all after-hours calls across Ohio markets, assessing urgency and coordinating appropriate response
  • Conduct comprehensive remote clinical assessments to determine patient and family needs, utilizing the nursing process to guide decision-making
  • Evaluate patient status changes, provide symptom management guidance, and communicate with physicians to obtain necessary orders
  • Support and direct on-call field staff by coordinating visit scheduling, bedside vigils, follow-up visits, and weekend care plans
  • Assist in developing and revising plans of care after hours in collaboration with the interdisciplinary team
  • Order and coordinate Durable Medical Equipment and medication refills, including required approvals
  • Monitor Matrix Care faxes and manage referral and intake-related communications
  • Enter referrals and admissions into the EMR and run benefit verifications as needed
  • Coordinate Docu Sign consents and ensure all required documentation is completed accurately and timely
  • Maintain detailed clinical documentation, including call logs, visit logs, symptom updates, and daily after-hours reports
  • Communicate clearly with case managers and interdisciplinary team members to ensure continuity of care
  • Participate in quality improvement activities as needed
  • Perform additional duties as assigned to support after-hours clinical and operational coverage
Job Qualifications
  • Associate’s or Bachelor’s degree in Nursing required
  • Active Registered Nurse license in good standing in the state of Ohio Required
  • Active CPR certification preferred
  • Minimum of 2–5 years of clinical experience preferred
  • Hospice experience strongly preferred
  • Triage experience required
  • Working knowledge of Medicare Hospice Conditions of Participation and CHAP guidelines preferred
  • Experience with Matrix Care or similar EMR system preferred
  • Strong clinical assessment and independent decision-making skills
  • Excellent communication skills with the ability to provide direction and support remotely
  • Strong organizational skills with high attention to detail and documentation accuracy
  • Ability to work independently in a fully remote environment while managing multiple priorities
