Scope of Position
This position is responsible for the post‑discharge coordination of follow‑up care provided to liver, kidney, and pancreas recipients after transplantation.
Position SummaryThe Comprehensive Transplant Center and all clinic locations are the point of entry for all transplant customers to the Ohio State University Wexner Medical Center. This role assists with coordination of all aspects of post‑transplant follow‑up care. The staff member will act as liaison between transplant surgeons/physicians, nurses, pharmacies, laboratories, home health agencies, and other support services. Responsibilities include providing patient education as indicated, assessing patient data to determine appropriate actions, and collaborating with the transplant team.
Minimum Education RequiredBachelor’s level degree or equivalent combination of education and experience with a major in nursing.
Required Qualifications- Graduate from an approved school of nursing.
- Current Ohio nursing license (BSN preferred).
- Prior transplant experience preferred.
- Current Certified Clinical Transplant Coordinator (CCTC) certification preferred.
- Flexible, sensitive, and adaptable to transplant activities.
Final candidates are subject to successful completion of a background check. A drug screen or physical may be required during the post‑offer process.
