RN Transplant Coordinator (Renal) – Ohio Health, Columbus, OH
Job Description SummaryThe Solid Organ Transplant Coordinator is responsible for coordination of the evaluation and preparation of patients for organ transplant and long‑term management of patients post‑transplant. This role coordinates patient care across inpatient and outpatient settings, serving as a clinical resource for team members, patients, and caregivers. The Coordinator collaborates with nurses, pharmacists, social workers, navigators, financial counselors, physicians, and community clinicians.
Responsibilities include direct patient education regarding transplantation, assessment, monitoring, and management of pre‑ and post‑transplant side effects and complications.
- Conduct pre‑transplant interviews with prospective patients and families; synthesize eligibility criteria.
- Analyze pre‑transplant clinical data and collaborate with medical personnel to prepare patients.
- Coordinate pre‑transplant preparations and evaluation processes.
- Educate patients and families on the transplant process and conduct post‑discharge planning.
- Provide direct care to transplant patients in the Outpatient Transplant Clinic.
- Assess post‑transplant organ function and overall health; determine need for additional intervention.
- Provide care in both inpatient and outpatient settings to facilitate continuity of care.
- Collaborate with clinicians to ensure patient needs are met.
- Use UNOS guidelines to select appropriate transplant candidates.
- Provide telephone consultation and follow‑up.
- Implement and coordinate education for live kidney donor selection and preparation.
- Stand on‑call during off‑shifts, weekends, and holidays to coordinate activity and provide triage.
- Participate in developing and implementing policies and standards of care for transplant patients.
- Provide education to patients throughout the transplant continuum.
- Assist in onboarding and training new associates.
- Participate in research and continuing education opportunities.
- Monitor and input patient data in collaboration with the Data Analyst to meet UNOS requirements.
- Complete UNOS documentation to maintain compliance (listing, status changes, post‑transplant follow‑up).
- Maintain relationships with external community stakeholders.
- Bachelor’s Degree in Nursing.
- Registered Nurse – Ohio Board of Nursing license.
Job Description
- Minimum 2 years of experience as a nurse in transplant or at least 5 years of clinical nursing experience.
