Registered Nurse - Case Manager

Job Overview

Oak Street Health is seeking a Registered Nurse, Case Manager to provide comprehensive care coordination for complex patients in its primary care setting. The RN, Case Manager will collaborate with the primary care provider, interdisciplinary team and community resources to prevent avoidable admissions and improve patient outcomes.

Columbus, OH – multiple clinic sites:
Deshler Park (1500 Lockbourne Rd), Eakin (Eakin Rd #100), North Linden (3096 Cleveland Ave).

• Manages an assigned caseload of complex patients, focusing on reducing admissions, readmissions, and medical utilization.
• Accountable for panel metric performance in admission prevention, readmission prevention, and transitions of care.
• Owns overall care coordination for assigned patients, acting as the primary point of contact and ensuring alignment across the care team.
• Manages transitions of care episodes, including completion of structured clinical assessments, medication reconciliation, and addressing identified needs.
• Collaborates with PCPs, family/caregivers, social workers, behavioral health specialists, and other team members to evaluate individual needs and plan of action.
• Ensures timely documentation of key clinical assessments and nursing interventions in the electronic health record.
• Leads in‑person interdisciplinary care planning meetings and provides patient education for chronic condition management.
• Performs other duties as assigned.

• Current RN license in assigned state.
• Bachelor of Science in Nursing preferred.
• Minimum 6–8 years of nursing experience; preferred experience in transitional nursing, emergency room, care coordination, discharge planning, or home health.
• Certified Case Manager (CCM) required, or willingness to obtain within 12 months, unless candidate has 2–3 years of relevant case‑management experience.
• Demonstrated skills in motivational interviewing, patient activation, time management, and navigating community resources.
• Flexible and positive attitude; comfortable with ambiguity and change.
• High emotional intelligence and ability to view situations from multiple perspectives.
• Reliable transportation and ability to travel between community clinics.

Hours:

40 per week (Full‑time).

Pay Range: $60,522.00 – $ (base annual salary).
