Schedule
  • Monday–Friday, 8 a.m.–5 p.m.
  • Only 2 holiday on-call shifts per year

What You’ll Do
  • Serve as the primary RN for your patient and family caseload
  • Complete assessments and determine hospice eligibility
  • Lead the admission process and develop individualized plans of care
  • Evaluate physical, emotional, social, and spiritual needs and coordinate interventions with the interdisciplinary team
  • Communicate with attending physicians, team members, and partner agencies to ensure seamless care
  • Conduct regular patient visits to reassess needs and update care plans
  • Perform clinical pronouncements of death and provide compassionate support to families
  • Maintain accurate, timely documentation, care plans, and progress notes
  • Supervise and document care provided by Home Health Aides per federal, state, and company guidelines
What You Bring
  • Graduate of an accredited registered nursing program
  • Current RN license in the state of practice
  • Current CPR and First Aid certification
  • Knowledge of quality assessment and assurance practices
  • Strong clinical judgment and ability to make independent decisions
  • Compassionate communication skills and ability to support patients and families
  • Valid driver’s license and reliable, insured vehicle
