** Scope/Position Summary
** The Clinical Case Manager facilitates an assigned group of patients through the health care system with the primary focus of efficient, effective, compassionate inpatient care and a safe transition to the outpatient setting. In some areas, this may include pre-admission to post discharge coordination of care. The Clinical Case Manager monitors the patient care process and physiological needs; documents, communicates, and collects data to evaluate and improve timeliness and appropriateness of care.

The Clinical Case Manager collaborates with physicians, nurses and other health care workers to provide comprehensive disease management assessment, treatment, and follow-up evaluation for patients and to communicate plan of care. The Clinical Case Manager maintains an awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization.

Minimum Qualifications

RN license and Bachelor of Science in Nursing required. 2 years clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations required. Prior case management experience strongly preferred. This role must understand the health care continuum and have the ability to solve complex problems and possess strong communication and interpersonal skills. Must be able to work with various computer applications.##

Additional Information:

Position will primarily support the Emergency Departments.

This role requires mandatory Week 1 training, Monday-Friday, on day shift. Attendance for all five days in the same week is required.##
