As a LPN, you’ll play a meaningful role in providing nursing services to individuals residing in our ICFs (Intermediate Care Facility). The LPN reports to a Registered Nurse and is responsible for working within the scope of their license to ensure that the physical health needs of the clients are met and maintained on an individualized basis in accordance with proceduresandcurrentfederal,state,andlocalstandards,guidelines,and regulations.

LPNwill coordinatehealthneedswith membersoftheclient's teamaswellasprovidenecessarytraining to team members. LPN Will also be responsible for delegating nursing tasks to certified direct service providers.

  18 years of age
• LPN License-State Boardof Nursing

• LPN License-State Boardof Nursing
• Ability to obtain and maintain First Aid, CPR - adult, child, and infant; delegated nursing. (training will be provided, if not already certified)
• Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles
• Ability to obtain and maintain physical crisis intervention training (as required by program).

40 Hours/Week;
Monday - Friday 8a-5p;
On Call Rotation and Floater between locations
