Job Description- Scope of Position – The position is responsible for the post discharge coordination of follow‑up care provided to liver, kidney, and pancreas recipients after they have received a transplant.
- Position Summary – The Comprehensive Transplant Center and all clinic locations are the point of entry for all transplant customers to The Ohio State University Wexner Medical Center. This position functions to assist with coordination of all aspects of post‑transplant follow‑up care. Staff members in this role will act as liaison between transplant surgeons/physicians, nurses, pharmacies, laboratories, home health agencies, and other support services needed.
They will provide patient education as indicated and must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age and acuity of the patients being served. The staff will assess data reflective of the patient's status and formulate an appropriate course of action in collaboration with the transplant team. - Responsibilities – Coordinate post‑transplant follow‑up care, act as liaison between consulting parties, provide patient education, assess patient data, formulate action plans collaboratively with the transplant team.
Bachelor’s Level Degree or equivalent combination of education and experience with a Major in nursing.
Required Qualifications- Graduate from an approved school of nursing.
- Current Ohio License; BSN preferred.
- Prior transplant experience preferred.
- Current Certified Clinical Transplant Coordinator (CCTC) certification preferred.
- Flexible, sensitive, and adaptable to transplant activities.
