8:00am – 4:30pm

Occasional travel to Dublin and Pickerington Trauma Clinics (Coverage support per business needs)

About the Role

We’re seeking a skilled and independent Registered Nurse to join our Trauma Resource team in a newly created role focused on care coordination, discharge planning, and patient education across the trauma continuum. This position is designed to support trauma patients as they transition from inpatient care to recovery, serving as a key connector between the inpatient trauma team and outpatient follow-up.

This is an excellent opportunity for an experienced RN who enjoys autonomy, collaboration with providers, and guiding patients through complex care plans rather than providing routine bedside care.

In this role, you’ll work closely with physicians and APPs to complete documentation, coordinate follow‑up care, and provide comprehensive education to patients and families. The role is less focused on hands‑on patient care and more centered on ensuring patients understand their recovery plan, next steps, and available resources. While this position is not assigned to a specific clinic, there may be rare occasions where coverage is needed at the Dublin or Pickerington trauma clinics if a Trauma Resource Nurse is unavailable.

You’ll be part of a highly collaborative, supportive trauma team that values trust, independence, and clear communication. This role offers the chance to help shape a new position within the program while making a meaningful impact on patient outcomes through strong education and care coordination. The ideal candidate will bring several years of trauma, ED, or ICU experience, strong clinical judgment, and a passion for helping patients navigate recovery beyond the hospital setting.

Responsibilities

and Duties
  • 45% Patient Care and Facilitative Functions
    • Screens and identifies patients.
    • Obtains complete health, social, environmental, and developmental history.
    • Communicates findings to other members of the trauma team.
    • Performs physical exam and evaluations related to injury and optimal function.
    • Assesses patient/family strengths and needs.
    • Interprets data obtained through a systematic assessment of body systems to differentiate normal versus abnormal findings.
    • Obtains clinical, laboratory, and diagnostic information; communicates and interprets results.
    • Initiates appropriate diagnostic, therapeutic, and educational interventions.
    • Designs and implements plan of care for patient and family, utilizing short and long‑term goals.
    • Evaluates and documents responses to interventions and modifies the plan of care as indicated.
    • Integrates changes as needed.
    • Establishes effective relationships with patient/family.
  • 25% Case Management and Collaboration
    • Provides case management to patient population with specialized needs at an advanced level to promote expert, innovative patient care.
    • Mentors other health care professionals, facilitating patient/family ability to assume responsibility for managing personal health care.
    • Ensures right services are delivered at the right site by the right provider at the right time for the right cost.
    • Assists with acquisition or verification of financial/insurance information when appropriate.
    • Works in collaboration with physicians to manage patients with acute and chronic illnesses within area of specialty, including obtaining history, performing comprehensive physical exams, and evaluations.
    • Facilitates and coordinates quality patient care in a timely, cost‑effective manner and promotes multidisciplinary collaboration among all members of the health care team, including initial trauma resuscitation, acute hospitalization, rehabilitation, and follow‑up in the Trauma Clinic.
    • Coordinates and mobilizes health care and community resources to meet needs of patients and families, including referral and consultation.
    • Analyzes inconsistencies and problems in the health care delivery system and influences changes to maximize patient outcomes.
    • Coaches members of the health care team to effectively promote optimum patient care.
