Riverside, Doctors, Grant, Dublin and Pickerington    Join our Maternal Fetal Medicine team as a Nurse Navigator! This role provides specialized care for high-risk pregnant patients with maternal conditions such as diabetes, hypertension and other complex needs. This salaried role offers a respected position within the department and a supportive team environment.
The Nurse Navigator will work exclusively at our Maternal Fetal Medicines sites, collaborating closely with multidisciplinary teams to coordinate patient care. Responsibilities include performing non-stress tests (NSTs), supporting in-office procedures and ensuring seamless communication between patients, providers and care teams. This role will be responsible for providing education and support to inpatients and outpatients. Responsibilities include addressing healthcare disparities and barriers to care for patients.

This position is responsible for providing education and support to inpatients and outpatients. Responsibilities include addressing healthcare disparities and barriers to care for patients; including facilitation of communication across the continuum of care, coordination of resources, education and advocacy, and coordination of care with managing physicians and the interdisciplinary team. May participate in some aspects of clinical care. This position exists in various departments throughout Ohio Health system of hospitals and healthcare services.

** 75%  Assesses learning needs, provides education, assesses learning achieved, and provides support and navigation to patients and families.  10%  :

May participate in some aspects of clinical care as needed.  5%  :
