Scope of Position/Position Summary
The Patient Care Resource Manager (PCRM) is a nurse case manager that facilitates patients through the health care system, including pre‑admission through post‑discharge coordination of care, resources, and services utilizing the nursing process. The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput, to enhance care quality and efficiency, and to ensure efficient resource utilization.
The PCRM identifies and removes barriers to progression of the plan of care for each patient.
Bachelor's Level Degree or equivalent combination of education and experience.
Required Qualifications- Bachelor’s degree, Masters preferred.
- RN licensure required; five years recent clinical experience applicable to the population preferred.
- Knowledge of clinical practice guidelines, computer databases, and payer protocols desired.
Final candidates are subject to successful completion of a background check. A drug screen or physical may be required during the post‑offer process.
