What You'll Do
- Coordinate client care services, including assessments, care planning, client visits, and caregiver introductions
- Supervise and support caregivers through coaching, orientation, quality assurance visits, and performance management
- Serve as a primary point of contact for clients, families, caregivers, and referral partners
- Collaborate with the Staffing Coordinator to ensure appropriate caregiver-client matches
- Maintain accurate documentation and support compliance requirements, including long-term care insurance documentation
- Participate in an on-call rotation to address urgent client and caregiver needs
- Work collaboratively across departments and perform other duties as assigned to support the success of our clients and team
What You'll Bring
- Minimum of 2 years of experience in a care-related field
- Previous supervisory or leadership experience
- Strong communication, relationship-building, and problem-solving skills
- Ability to lead, coach, and hold team members accountable
- Valid driver's license, proof of insurance, and ability to travel locally
- Basic computer proficiency, including Microsoft Office and data entry
- Ability to successfully pass a criminal background check
Schedule
- Full-time position
- Monday-Friday, 8:30a.m.
- 5:00 p.m. - Participation in a shared evening and weekend on-call rotation
- Some flexibility required based on client needs
