Capital City Hospice is seeking a Hospice Transitional Care Navigator. Competitive salaries, 401K + match, and so much more!

Location: In the assigned market (Columbus, OH).

Salary: $92,000 - $105,000 Annually

This position is responsible for collaboration with the Hospice Liaison team and Admissions team to increase public awareness of hospice and other services provided by the agency and to drive admissions for the placement of patients to meet expectations. The role serves as the first point of contact for hospitals, health care facilities, and other sources regarding referrals, seeks networking opportunities for education, and develops relationships with new and existing referral sources.

Additionally, the role identifies appropriate care and services for patients and families in accordance with attending physicians, plans of care, and company policies and procedures.

Position Requirements :
  • Bachelor’s degree in marketing, business administration, or other related fields is preferred; acceptable combinations of experience and/or training may be considered instead of formal education.
  • Licensed as a registered nurse in the state of practice, a plus.
  • Must possess the ability to make independent decisions when circumstances warrant.
  • Must be able to deal tactfully with patients, family members, visitors, agency personnel, and the general public.
  • Must be knowledgeable of quality assessment and assurance procedures.
  • Valid driver’s license and proof of insurance are required.
