Registered Nurse, Care Coordinator – Post Covid Care

Responsibilities

• Collaborates with clinic team members to provide evidence-based patient-centered care
• Helps patients stay up to date on care plans and related goals
• Focuses on high-risk medically complex patients, including those in care transitions
• Acts as a liaison with patients’ family members and other agencies
• Facilitates improved communication to provide comprehensive whole-person care
• Assists with programs and processes, staying up to date on standards

• Associate Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program
• Current Ohio Registered Nurse license
• CPR/BLS certified
• Knowledge of triage procedures along with clinic, physician office and/or urgent care experience
• Ability to maintain quality, safety, and/or infection control standards
• Knowledge of appropriate procedures and standards for the administration of medications and patient care aids
• Ability to educate and clearly communicate with patients and/or families as to the nature of disease and to provide instruction on proper care and treatment
• High level of interpersonal skills to handle sensitive and confidential situations
• Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions

Demonstrates expertise in evidence-based patient-centered care, focusing on high-risk medically complex patients. Proficient in triage procedures, medication administration standards, and effective communication with patients and families.
