In-Home Nurse Practitioner or Physician Assistant - Indianapolis

Job Summary

Signify Health, part of CVS Health®, is looking for a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform In‑Home Health Evaluations, engaging with patients in their homes to bridge care gaps and provide personalized assessments.

• Deliver 45‑minute home visits, including medication and medical history review, physical evaluation, and optional diagnostic tests.
• Use an iPad and intuitive clinical workflow for each evaluation.
• Maintain professional standards—no prescribing, ordering lab tests, or changing treatment regimens.
• Collaborate with Signify Health clinicians and meet role‑specific requirements.
• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits when necessary.
• Provide Diagnostic & Preventive Services such as spirometry, DEE, bone density, etc.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses preferred.

Typical pay range: $78,331.00 – $. Actual base salary depends on experience, education, geography, and other factors.
