Registered Nurse, Care Coordinator – General Internal Medicine

Responsibilities

• Collaborate with clinic team members to provide evidence-based patient-centered care
• Help patients stay up to date on care plans and goals
• Focus on high-risk medically complex patients, patients on disease registries, and care transitions
• Act as a liaison with patient’s family members and other agencies
• Facilitate improved communication for comprehensive whole person care
• Stay up to date on standards, assisting with programs and processes

• Associate Degree in Nursing from a CCNE or ACEN accredited nursing program
• Current Ohio Registered Nurse license
• CPR/BLS certified
• Knowledge of triage procedures
• Clinic, physician office and/or urgent care experience
• Ability to maintain quality, safety, and/or infection control standards
• Knowledge of appropriate procedures and standards for administration of medications and patient care aids
• Ability to educate and clearly communicate with patients and/or families regarding disease nature and proper care and treatment
• High level of interpersonal skills to handle sensitive and confidential situations
• Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions
